Usage Instructions:
There is no fixed dosage regimen for the management of hyperglycemia in patients with type 2 diabetes using Metformin XR.
Metformin XR dosage must be individualized based on both effectiveness and tolerance, while not exceeding the maximum recommended daily doses.
The maximum recommended daily dose of Metformin XR for adults is 2000 mg.
Metformin XR should generally be administered once daily with the evening meal.
Metformin XR should be started at a low dose, with gradual dose escalation, to reduce gastrointestinal side effects and to allow for the determination of the minimum dose required for adequate glycemic control in the patient.
Adults:
The usual starting dose of Metformin XR (metformin hydrochloride) extended-release tablets is 500 mg once daily with the evening meal.
Dosage should be increased in increments of 500 mg weekly to a maximum of 2000 mg once daily with the evening meal.
If glycemic control is not achieved with Metformin XR 2000 mg once daily, a trial of Metformin XR 1000 mg twice daily should be considered.
If higher doses of metformin are required, metformin should be used in total daily doses up to 2550 mg in divided daily doses, as described above.
Warnings and Precautions:
Risk of Lactic Acidosis:
Metformin hydrochloride may cause very rare but very serious side effects called lactic acidosis, especially if your kidneys are not working properly, if you have uncontrolled diabetes, serious infections, liver problems, or dehydration.
Stop taking metformin hydrochloride and contact a doctor or the nearest hospital immediately if you experience symptoms of lactic acidosis, as this condition may lead to a coma.
Symptoms of Lactic Acidosis:
- Vomiting
- Stomach pain
- Muscle cramps
- General feeling of discomfort with severe fatigue
- Difficulty breathing
- Low body temperature and heartbeat
Risk of Hypoglycemia:
If you take metformin hydrochloride with other diabetes medications, it may lead to hypoglycemia (low blood sugar).
If you experience symptoms of hypoglycemia such as weakness, dizziness, increased sweating, rapid heartbeat, vision disturbances, or difficulty concentrating, eating or drinking something containing sugar usually helps.
Possible Side Effects of Metformin Hydrochloride:
All medicines can have side effects. Sometimes they are serious, but most of the time they are not.
You may need medical treatment if you experience certain side effects.
Tell your doctor if you notice any of the following and they worry you:
- Lactic acidosis which can lead to coma
- Digestive system problems such as nausea, vomiting, diarrhea, abdominal pain, and loss of appetite
- Change in taste
Other Medicines and Metformin Hydrochloride:
Tell your doctor if you are using, have recently used, or might use any other medicines.
- Medicines that increase urine production (diuretics).
- Medicines used to treat pain and inflammation (NSAIDs).
- Certain medicines for high blood pressure.
- Beta-2 agonists such as salbutamol (for asthma).
- Corticosteroids.
- Other medicines used to treat diabetes.

Each XR 500 extended-release tablet contains 500 mg metformin hydrochloride equivalent to 390 mg metformin base.

Each XR 1000 mg extended-release tablet contains 1000 mg metformin hydrochloride equivalent to 780 mg metformin base.
